In order to view the forms, you will need Adobe Acrobat Reader. If you do not have it installed already, you can download the program by clicking on the following link: http://www.adobe.com/prodindex/acrobat/readstep.html To have forms sent via U.S. mail, please email us at customercare@sbliusa.com or call us at 1-877-SBLI USA (1-877-725-4872).

 
I never received a premium notice. What should I do? ↑Top↑
Please verify that we have the correct address on file for your policy. Go to 'Service'. Select 'Address Change' to view the current address and make any necessary updates.

Then, you can make your payment online with a credit card or you can have funds electronically transferred through our website. See How to Make Online Premium Payment.

In order to view your premium amount due, please enroll and login to our online Customer Center. Click on your policy number to view your premium information.  
How do I make an online premium payment? ↑Top↑
Payment can be made using a credit card or you can have funds automatically transferred from your checking account. In order to make a payment online, you must first enroll and login to our online Customer Center. Once you have logged in, please click on your policy number, then click on the tab entitled "Service." Select "Make a Payment," and follow the instructions to make your premium payment. If you wish, you can also set up the payment as a "Recurring Payment."

In order to view your premium payment history, you must first enroll and login to our onlineCustomer Center. Click on your policy number to view your last premium paid date.  
How do I change my billing mode? ↑Top↑
Enroll and login to our online Customer Center. Click on your policy number, then click on the tab entitled "Service". Select "Change My Payment Schedule", and follow the instructions to request this change.  
I cannot locate my original policy. What should I do? ↑Top↑

Enroll and login to our online Customer Center. Click on your policy number, then click on the tab entitled "Service". Select "Duplicate Policy or Certificate", and follow the instructions. 

 
How do I file a claim? ↑Top↑
Go to the home page by clicking on the SBLI USA logo, then select "Log In," then "Claim Forms," and follow the instructions to file a claim.  
What are the dividends options available on my Whole Life policy? ↑Top↑
Please refer to the following Dividend Options available on Whole Life policies: (1) Cash (2) Reduce Your Premium (3) Purchase Additional Paid-up Insurance (4) Accumulate at Interest (5) Purchase One-Year Term Policy  
My name has been legally changed, how do I change the name on my policy? ↑Top↑

Enroll and login to our online Customer Center. Click on your policy number, then click on the tab entitled "Service". Select "Name Change", and follow the instructions to request this change.

 
How do I change my dividend option? ↑Top↑

Enroll and login to our online Customer Center. Click on your policy number, then click on the tab entitled "Service". Select "Dividend Option Change", and follow the instructions to request this change.

 
How do I change my beneficiary? ↑Top↑

Enroll and login to our online Customer Center. Click on your policy number, then click on the tab entitled "Service". Select "Change My Beneficiary", and follow the instructions to request this change.

 
How do I change my address? ↑Top↑

Enroll and login to our online Customer Center. Click on your policy number, then click on the tab entitled "Service". Select "Address Change", and follow the instructions to request this change.
